SUPPLEMENTAL INSURANCE
If you are injured during a game please let us know. We care what happens to you! Remember, your annual $30 NCASA/USASA member fee covers a $5,000 supplemental accident policy. Supplemental insurance covers gaps in your medical coverage, and may help pay for medical costs up to $5,000 that are not covered by your primary policy.
If you are injured during a game, we will provide you with the match report, which includes the time of the injury, your name and/or jersey number and a brief description of the incident. The match report serves as evidence for your claim. Claims are filed only online.
Once you have the game report, visit USASA to fill out the claim form. Submit the form as soon as possible to get the process started. Please also feel free to reach out to our state office with any insurance questions. Darius and Cindy are wonderful to work with, and they are here to help.

During times of potential or existing inclement weather, the Weather Hotline is updated for athletic games and practices at the following times:
- 4:00 pm on weekdays
- 8:00 am on Saturdays
- 12:00 pm on Sundays
